Semi-Duplicate Results

Greetings!

I upgraded to a version of WP 4.2.X (not sure which verison) which through the plugin out of whack.

Im basically getting results that show this:

Im gaving a bugger of a time trying to find where the coding would exist to see whats going on, BUT does anyone have ANY clue what’s going on with the duplication?
