Change profile avatar is an activity trigger from the BuddyPress integration and it only get triggered when an user changes his avatar through BuddyPress
About fields you mention on steps, I have a fix for It and will come with the next version (1.1.5)

Actually the trigger doesn’t work even when changing the avatar through Buddypress. Changing the title picture does cause a trigger, changing the avatar doesn’t.

Yeah! I finally got this thing running! 😀
But I still figured out some little bugs. After I renamed an achievement type it disappeared in the profile list (even the tab with the name on it). Then I rewoke the award and set everything new. After I did this I found out something strange. Here is what I did:
Achievement data Type
Singular name: Identität
Plural name: Identitäten
slug: identitaet
After the achievement was awarded I took a look in the profile and got an error because the link was: mitglieder/spickzettel/verdiente-orden/identität/
So it didnt take the slug for the link but one of the singular or plural names. I got this working now by simply naming everything the same. But this should be fixed.
But hey, I love this plugin anyway. You are doing an excellent job. If there is ever gonna be a pro version, let me know! I would be interested in shortcodes that can show different content in relation to achieved awards. For example:
NO AWARD A => Show “Hey get your first award now by doing this!”
AWARD A EARNED => Congratulations! You are a award a now. Here are some next awards you could earn.
