Questions Tab completely empty

I installed the plugin and everything seems to work.
Only BIG problem is that when i edit a quiz, the “Questions” tab is completely empty (blank page), other tabs work fine. What could it be?

Thank you for using QSM! On the Questions tab, do you see the create new page button and text or is the entire tab empty?
The 500 error listed suggests an error from the REST API. Would you be able to perform a plugin conflict check? This process will check to see if there is a plugin that you have installed that may not be compatible with the plugin that you are trying to use. To do this, first, de-activate all other plugins besides Quiz And Survey Master. Now, check to see if it is working correctly.
If this fixed the problem, this suggests that it is a plugin conflict issue. To discover which plugin may contain the conflict, activate one plugin and then check if the plugin you are setting up still works. If it no longer works again, the plugin you just re-activate is the plugin conflict. If not, continue to activate the plugins one by one and check to see if the problem arises again.
If you do find a plugin conflict, please let me know which plugin and which version of the plugin that you are using.

I have performed the plugin conflict check, deactivating plugins one by one, but still have the problem.
One clarification, I don’t have any error in console. The tab just doesn’t load anything, the content of the tab is empty:

Thank you for completing the plugin conflict check. Hmmm… It is interesting that the tab is entirely empty.
Can you paste some of the system information from within the help page? To do this, please go to the “Help” page in the “Quizzes/Surveys” menu. On that page, copy everything in the “System Info” section and send it to us in your reply to this email. The system information includes data that helps us eliminate a variety of issues and narrow down what we need to look into.
You can leave out the URL part of the information. I just need the server/site info to see if I can create a similar site set up to try to replicate the issue.

Thanks for posting the system info. I see that your WordPress version is 4.5.3. We only support versions 4.8 and 4.9. That said, if you are encountering the same error that @acbrent25 posted, it may not be a WordPress version issue.
Are you able to turn on Debug mode? On some hosting providers, there may be an option to turn this on. If you are technical, this can be done by following this page. If so, can you turn it on for long enough for you to check the “Questions” tab to see if any errors appear there or in your error log?
