How can I tell if my theme or other plugins are managing open graph images?
You’re not using any other Open Graph plugin at the moment, and Jetpack seems to output the tags properly:

As you can see, it also includes an image tag using your post’s featured image. That’s just the way it should be, and there isn’t anything more you can do on your end.
Facebook Debugger is able to see the image:

Error parsing input URL, no data was cached, or no data was scraped.

Sometimes yes, although I cannot really explain why.

Sometimes this happens when Facebook cannot grab any info from the page ecause it takes too long to load. It might be worth looking into your posts’ first load speed if that happens again.
