I’d imagine that Extended adrotate plugin does something weird with the code.
Otherwise I can’t say much with so little information.

This is URGENT issue.. I have installed Adrotate in my demo server.. 2 weeks ago .. and everything was working fine.. Today I installed in on live server, Latest version of Adrotate through search and install, and also through uploading zip file. But add new page shows “New Ad”. Below that its empty. I inspected it. It showed these errors.

This look more like things from your access_log than errors.
Check your error_log for some actual errors mentioning AdRotate.
