It is a bit hard to answer why you are experiencing this issue
The best advice I can give is the standard fault finding and fixes.
I first advise you to clear the cache and cookies. You can also try to reset your permalinks and try again.
The issue could also be due to a theme or plugin conflict
Themes:
As a test, switch to a default theme such as twenty-twenty and see if this fixes it.
Plugins:
If that doesn’t work, also deactivate all plugins apart from QSM and see if this fixes. if it does, re-enable one at a time to see which is causing the error.
If you cannot do this to your site (say because it is live) then use the ‘troubleshooting’ features of this plugin to let you test without affecting other users

I followed your instructions by installing the “Health Check”. In Troubleshooting Mode, with only QSM active and with the standard theme we have the same problem.
It happens when you click on the form fields.

It seems that you may have accidentally added a mailto link somewhere on the WordPress page.
I do not know where this is added as it is not visible on the front end.

It could be a widget before the QSM quiz, or it could be within the same widget that you used to add the QSM quiz shortcode.
If you are not able to find the mailto link and remove it, I would suggest you please deactivate the third-party plugins and then clear the cache.

Hi @sumitsanadhya, yes you’re right. I just check there was an old email link in the page from the previous text that we added there that doesn’t appear but create the problem. Now it solved thanks!
