Could you link to the page that you you’ve attached the form to so I can take a look at the source and see if Ninja Forms is even initiating on that page at all. This is just a start but might help me to troubleshoot your particular issue.
Ok, I’m not very familiar with the Genesis framework as I don’t have a copy to test with. Ninja Forms applies a filter to the_content() to display the form after a page or post and so either Genesis is overriding that filter or not directly using the_content() in the theme.
Try using the shortcode provided for the form in the edit page screen editor and see if the form shows up with that.
Had already tried shortcode, but no joy.
Have removed all traces of Genesis framework & themes, removed all plugins except Ninja Form Lite, Akismet, and changed theme to Automattic’s “Toolbox” — a pretty vanilla setup, but still nothing.
Browsers tested in: (Mac) Safari, Chrome, Firefox, (Win) Chrome, IE8
Also of note, trying to drag & drop to rearrange form fields results in repeating drop target “outlines” in all browsers. See screenshot: <http://cl.ly/0M082R2h2g3x370C0J3u>
Only other option at this point is to install a fresh WP 3.3 & test from there. Won’t be able to do until later this afternoon. Will follow-up then.
Jim – I am having the same problem, however, when I click on your link I can actually see your form.
I receive the same “Because of different CSS settings, this preview of your form may appear different than what your end-user sees” message. Using the shortcode I can’t get anything to appear either.

Phil – I think the comment form you’re seeing on that page is the standard WP comment form. The Toolbox theme doesn’t provide the option to turn comments off on pages. I can see it myself if not logged in as admin (comment form that is), but no Ninja form.

1.2.7 *should* appear shortly in the repo, so if you have the plugin installed, you will be able to update from the “Plugins” admin menu within WordPress. 1.2.7 fixes the bug that I caused by not being able to type.

Like a dolt, I deleted my 1.2.6 install since it thought you might not bump the version number. Went looking for 1.2.7 in repo to install fresh, and it’s not there (http://wordpress.org/extend/plugins/ninja-forms/).
