[Plugin: Jetpack by WordPress.com] Contact Form does not display on page

I am using JetPack v 1.8.2 and the Genesis+AgentPress theme, all current. Other than the Contact Form, I am using JetPack only for “Image (JetPack)” to display an image in primary sidebar for a hyperlink. Clicking on the icon above the page-edit window to insert a form, everything seems to work just fine, including adding a “Phone” field. When I click “Add this form to my post” … yes, post, not page, and I tried it for a post with same results … the following code is inserted … same code in display window in both “Visual” or “HTML” mode …

but after “Update” no part of the form displays – only the page title with nothing below. Here’s a link to that page
