Optin Form looks different

As of an hour ago, my optin form looks different from what it originally looked like after I updated the plugin. Something looks broken. I attempted to use all of the different number forms 1-6, and they are all messed up. Here is the site: http://www.jetsetandforget.com. I was using form 5.
