One form has stopped displaying

One form I use, actually my first, so #1, has stopped working, others appear to be working. The form simply doesn’t display at all. Text on the page does appear. Switching to “Direct” mode doesn’t change this. If I deactivate CFtE the code shows as text as you’d expect. If I reactivate CFtE I get back to same no show. If I go to the form settings the form seems to have reset to the default with the “awesome” message. I can edit the title and the message but the fields I had have gone and adding a field simply doesn’t work: doesn’t do anything. I have tried cloning the form and it’s the same. I have tried cloning another form that works and that clones and works. I’m really baffled. The page is at: https://www.coresystemtrust.org.uk/contact-us/ I’m not sure how long ago this happened but I’d guess quite recently, i.e. in the last few weeks. The last time I can see I got a message from the form was 2/6/16. WP shows no changes to the page that calls the form in over a year (or did, until I tried editing the form, which worked but didn’t change any of these problems). If I change the number of the CFtE form to another in the page then I get the other form and it works. Only other odd thing is that the icon to insert a CFtE form in the page editor has disappeared. Some plugin clash? Feels a very bizarre one.
