not working on custom theme

    Hello
    the pop up is working fine on wordpress default theme but it is not working on a theme created by me.
    I can see the code when I load the page but the pop up is not there, I have wp_head(); called on my theme.

    Any idea what is causing the problem.
    I have disabled all plugin and script and still no success
