Login shortcode broken/2.6.4 update broken

I have paid for and installed the pro version of Front End Only Users. The login shortcode at http://mcclellandsbeach.com/login/ does not work. All I see is the code text. Also, when I try to click on the offered update 2.6.4, it does not take. Thank you.
