The Application Page contents are determined by how you set-up the Application
via the (App. Form Setting)s and any Mods or Conditionals you establish
on the (Display Settings Page.)
There have been several discussions of late regarding the Page Builder/Visual Composer. At present we don’t support those Theme types. This means you may have to mod your style.css CSS to get the look you desire.
The styles you should copy into your css.style dealing with Job Manger are located: job-manager/css/display.

I have set up these things. However the application page http://www.kynniblakeinc.com/jobs/apply/1633/ is still formatted differently than the job page http://www.kynniblakeinc.com/jobs/. I didn’t need to do anything to format the jobs pages, it shown up correctly on own.
Can you look at the pages via the links above and please provide better assistance or explanation of how to fix it?

Ok, I see the issue. The Theme your using uses Visual Composer which has a conflict with Job Manager. Unless you can get the Theme Builder to make suggestions there is not much we can do.
I can tell you that we have reached out to the Developer of Visual Composer and asked for assistance as well as a Licensed Dev Copy to test with.
