how to add an additional field?

    Hi, currently i’m hooking up my mailchimp with the plugin and have been using for more than 3 months now. Love it!

    My form at mailchimp has 3 fields: name, email, phone number
    How can I add an additional field to collect the phone number as the same in the plugin?
