Getting Duplicate Entries

We have Formidable forms setup in one of our sites for quote requests. Occasionally, the shop manager sees duplicate emails come in for an old entry. There were 2 reported today, which was confusing since those customers already sent their request (and were contacted/resolved) weeks prior. When I checked the actual Entries list in Formidable/WP Admin, it does show an entry which explains what triggered the emails. But the shop owner is claiming this is a duplicate entry, and the original happened weeks ago. Is there any known issue of entries being duplicated?
Other notes: I have checked that WordPress and Formidable are all completely upgraded to the latest versions. I have checked the email settings are working and unchanged. We have not been making any changes to the forms or form settings recently. The owner claims this happens regularly, at least a few each month. There are no cron tasks running on the server. To my knowledge, the customer did not place the form submit again.
