I’ve been able to replicate the issue here. Could you try duplicating the form, removing any email notification from From > Edit > Email Notification > Delete all the notification, and then applying the form on the new page?
When we submit a form, I suspect it fails to trigger the email because the email configuration appears to be malfunctioning.
If the above workaround doesn’t help narrow down the issue, you can share two things with us that can help investigate this case further.
– Export the form and share it with us? Please refer to this doc to learn about the export form method:- https://wpmudev.com/docs/wpmu-dev-plugins/forminator/#import-export.
