That means our plugin cannot create a custom table (cr_local_forms) in your database to store the review form information. Please contact your hosting support and ask them for the correct permission to create a table in the database.
If you can’t create a table, does it mean it’s due to hosting?
I checked in my database, it still creates the table as above but there is no data

Can you please share a screenshot of the table structure? You can try to deactivate all plugins except CusRev, WooCommerce and switch to a standard theme of WordPress and check this issue again.
