Could you please confirm:
- Is your custom field created as an Image field type (for example in ACF), not just a text/URL field?
- Are you using the exact field name (meta key) in the plugin settings?
Also, please make sure you are using the actual custom field name (my_image_field) in the setting. In many cases, ACF stores an additional internal field that starts with an underscore (for example _my_image_field). That internal key usually should not be used here. Please double-check that you are using the main field name instead.
Please double-check the field type and the exact field name. It’d work once the correct image field name is used.

I use Featured Image from URL or FIFU but still it will be taken first image
Unfortunately I am not firm with custom fields and ACF, understand nothing what you write about this
But there are remote fatured pictures with this 2 plugins. It should work with that.

Corrected , it will only be default picture from plugin shown, not first image as I remember

Thanks a lot for explaining your setup so clearly.
You’re right, at the moment, Related Posts Thumbnails does not support remote/URL-based featured images (such as the ones added by Featured Image from URL / FIFU). It currently works only with locally stored images or image-type custom fields.
This is a great feature request. Thank you for sharing it with us. We really appreciate the idea.
We’ll add a new option in the plugin that will allow using an image URL field (remote image) as the thumbnail source, so users using FIFU or similar plugins can display their thumbnails correctly.
Thanks again for the suggestion. We’ll ship this feature in an upcoming update.

Is it possible to send users to my standard login/register page, not to standard wp-admin. Normally wp-admin should faward to it
There’s no such option in the RPT plugin. You can try LoginPress for a complete login system solution, or alternatively, use the WordPress redirect hooks as a code-based approach.
