    Hello @johnosmond,

    Thanks for reaching out to us!

    First you need to go to your WordPress dashboard > Testimonials > Form > add there the fields you need. After this you need to go to your WP dashboard > Testimonials > Views > edit your view> Fields > Custom fields > add there your fields > Save changes.

    You need to copy the shortcode of your view and paste it on your page and that’s all.
