First of all, Go to contact page template on your localhost site where you are working and remove shortcode here.

Then you can use any type of form as per the requirement.

Hi Shahid,
I tried to edit the contact page template as per the printscreen you sent me, but the only visible form is the one I created and pasted. There is no other shordcode regaring the form. I don’t know what to delete.
Any other suggestions?

Kindly tell us are you using the theme of Appointment Pro. Because we don’t provide contact page template support in our Appointment lite theme. If yes kindly create your query on our support channel. Because WordPress does not allow us to handle premium products queries.
