If there is a server-cache enabled, like an Object or Varnish cache, please flush it and see if the issue will be resolved.
I’ve tested a preview on my end and could not detect any issues with the appearance. Would you please run a quick test for a conflict with another plugin?
– Deactivate all plugins, besides the Forminator and see how the form preview looks like,
– If the issue persists, switch to a default WordPress theme,
– If all is well, activate one plugin at a time, every time check the preview of a form, if it is showing up well.
Just in case, please run a full backup of your site, so if something will go wrong, you can always restore it.

@wpmudev-support9 Thank you for your response, I deleted cache and it went back to normal.
One last question there is a form I can’t find on the backend that is supposed to be from Forminator but it displays on the site (with a forminator ID) but not on the plugin on the backend. I also checked the Contact Forms and it doesn’t belong there either. Are Forminator forms save elsewhere on the backend?

I apologize for the late reply in this thread. All forms that are created with the Forminator plugin should be at Forminator > Forms. Would you please share here a link to the page with the form?
If the page will be edited, is the forminator shortcode added on a form?
If it is not showing, it is a strange issue. Would you please check the form ID and look for this ID on the wp_posts table, and in wp_postmeta look for an entry with the post-id=40. Please remove both records and see if the issue is resolved. Just in case, please backup the database of your site, so it can be restored if something will go wrong!
