Booking form is not working.

Your plugin functionalities are great. However i am finding it difficult to make any modifications to the default form. I would really appreciate your help.

I tried to add two fields as First Name and Last Name and dragged in 2 fields to the form. After providing the FirstName as the label for the first field it is getting saved appropriately. But when i add the 2 field name as Last name the first field’s Label too is getting changed to Last name. I would appreciate your reply.
Yet another issue i encountered is to do with the Check-in and Check-out on the calendar. I have enabled it on the settings page but it is not getting displayed on the actual webpage.
