Blank Admin page

I’m not able to get this plugin to work on my install of WP (4.1.1) with the latest Any Lead (1.1).
I’ve disabled plugins, changed to a standard theme (Twenty Fifteen), and I’ve downloaded directly from WP.org and installed via admin panel, just to be sure. But everything I do leads to a blank admin page when I click on the WP Any Leads link in the admin panel.
