    Is there a way to add additional fields to the review form?

    Adding additional fields to the form is not actively supported.

    However, if you are adventurous and would like to give it a go on your own, please see: https://pastebin.com/4fvKErcp

    One more question. These instructions also indicated that a review.php template needs to be created, and then references the plugin FAQ section to find out more about how to do so. Where’s the FAQ section?

    1. Log in to the WordPress admin
    2. Go to: Site Reviews > Documentation > FAQ
    3. Read the “How do I change the order of the review fields?” section
