Add-on Cost not populating

The above link is a sample of what I am working on. I have added one Add-on section, and two of the options are supposed to have pricing increases. Please help me identify what is wrong?

The backend setup includes the pricing increases, as you can see from the screenshot below; however, on the product page, only the radio buttons and options populate.

I have gone through all the settings and cannot find anything that says “show add-on price on product page” or anything like that, so I am assuming it is supposed to automatically populate. I did not receive an automatic account profile upon adding the plug-in to my site, so maybe that is the problem? There is no option for a license either, as I am using the free version.
