Services review plugin?

Hi, I was wondering if anone knows of a review plugin that is very customizableflexible. What I’m looking for is:

Option to have tick boxes for services, so as for example, when reviewing a restaurant I want a checkbox available for; accessibility, child friendly etc. So a binary response.
Also it should be visible in a post a user adds. Be it through a link or button that pops up the review or automatically add it to the post itself….

I have seen something like that for vBulletin. In the post there would be the text ‘ Review’ and if you clicked on that the review data would pop up. After the word review was an overall rating. Looked very cool and helpful.

Anyone know of such a plugin?
