[Plugin: Custom sidebars] Javascript error with accessibility mode

Hello,
I am using Custom Sidebars 1.1 and I noticed there’s a problem when adding a widget using accessibility mode (enabled via Screen Options), it triggers the following javascript error:
Uncaught ReferenceError: pp is not defined
on the file
/wp-content/plugins/custom-sidebars/cs.js?ver=3.4.1
This error prevents my TinyMCE Widget plugin from working correctly (see this thread for further info).

Moreover I found another small bug on icons that have wrong urls:
