I have the same problem, i do get a fatal error when i try to install the plugin again.
I uninstall and reinstalled the plugin but it still not working.
I hope i did not lose my counters, i did got more then 100000.
Want to know how to fix this issue and if i did lose my counters how can i add them back true database ?

Hello Sir.
Can you guide me and maybe share your modified files and let me know where to place exactly to make it working.
I have try`t everything and it does not save the new added fields in the user fields.
Maybe you can write a small easy guide to help.
I actually only need 2 extra fields like First Name/ Last Name/ , i followed the guide here http://docs.thememylogin.com/ but i can`t get it to work ;(
