Add &d=http://url-to-default-image.jpg to your url or use RecentComments it comes with build in gravatar support.
Switch to this Plugin RecentComments. You can configure the max length of the titles via the admin panel.
I suggest you to use the RecentComments Plugin. It comes with gravatar and widget support and tons of other features.
