Thanks for your answer. I tried to duplicate one of the forms, and the copy worked well but not the original. I have no idea where the problem came from, but your plugin works well.

I found the problem : Global Site Access was set to Logged In users, now it’s open to everyone and it works. I will find an other solution to block the website to users that aren’t logged in.
