This is how I got it to work:
Download and install version 4.0.5 of this plugin. (I can’t get the latest version to work.)
Find an image of the flag you want to use. (I did a google image icon search to find one.)
Size it to 24×24 pixels using graphics program and name it English.png
Upload the new image to google-language-translator/images/flags24.
Now view the page on your website where your flags are.
If the new flag isn’t showing do a ctrl f5 to refresh.Hope this helps.

Try creating a new user with admin privileges and log out and log back in as the new admin.

Wow! I think that did it. I would have never thought of it. I will give it the rest of the day and then mark this as resolved by you.
Do you think I should remove those extra lines I put into wp-config file ?

I don’t know if this will help your or not – but I had exactly the same problem. It was resolved by disabling the Google Analytics Suite plugin. Of course now I have a new problem. 🙂
