I had the same issue and found out it was caused by the Login Widget With Shortcode plugin. Deactivating that plugin allowed me to access the settings tabs on Pirate Forms again.
I am having the same issue. Click sign in button, sign in, authorize the app, it then redirects me back to the WordPress dashboard but it is not showing me as signed in and tweets are not automatically sent.
