Hi,
Just leave the admin message blank to get the default table view of all data submitted.
You can use placeholders in your email. For example if you have a field called email_address then add {{email_address}} in your email body to have the submitted email address displayed.

Please make sure the address in question does not have specific settings allocated to it. If one gmail address receives it and another doesn’t it can only mean differences in the email account setup.

You have a javascript error originating from another plugin or the theme you are using. Other then that is that the base path my differ from your WP->settings site URL (this I have seen only once before) usually its a javacript issue.
