I experienced the same problem and after much research discovered that it was due to an update of the wp spamshield plugin. After disabling that plugin, and resetting the connection to my wordpress.com account, everything is working fine again. I have put in a request for support to the wp spamshield team since that is a great plugin.

Does anyone have a working version that allows conditional display of form data? I simply want to display the form data of the current user but the conditions entered on the front end do not show up in the tinyMCE editor and putting them in manually simply suppresses all output except for the column headers. Unfortunately, I am not good enough with PHP and mySQL to debug these errors.
