Fields are to be added at the Settings page at MailChimp. Once you are set, you’ll see the list of custom fields within the plugin.

Seems like either api, or the list is not set.
Can you try the following.
1. make sure your API is set, an then Click Get Lists button.
2. in the Custom Forms section, select the ListThank you,

It might be due to some errors in JavaScript in the website. Please see if there are any errors shown up in the Console area of your browser.
