I have same problem with really simple captcha. I am using wordpress Version 3.5.2, Really Simple CAPTCHA Version 1.6 and Gravity Forms Version 1.7.6.
On local server it is working fine but on live server the captcha image is not displaying.
WordPress is installed in a sub-folder on server.
I have re-installed the Really Simple CAPTCHA and also installed lower Version of Really Simple CAPTCHA i.e. Really Simple CAPTCHA Version 1.4, modified the .htaccess file in upload folder but no luck.
The GD and freetype are there and permission is also 775 for upload folder but not displaying the captcha image. It is redirecting to 404 page when I open the broken image’s URL i.e. the image is not on the server. When i check the upload folder the images are continuously generating either i submit the form or not.
