Try to setup your url to not have index.php.
May be this is the reason.
Plugin is tested with nice urls and with urls like index.php?post_type=… but not with urls like index.php/test/…

Indirectly this possible by transforming each answer into question with 4 answers.
So where you have 1 question with 4 answers there will be 1 section with 4 questions which have 4 answers each.
Do you have non-standard theme or other plugins active?
@jasaldivara, how this 404 page looks like — as a worpdress page or as a webserver page?

Also this plugin does not have functionality to “randomly reorder answers”. It could be caused by conflicting plugin or loose theme developers.

Not wp-testing and wordpress contains any limits regarding 500 questions.

@shawnswick can you show some screenshots, that you have questions, answers and scales? Have you saved the test?

In reply to: [Psychological tests & quizzes] Scales keep reorderingSeems like some other theme/plugin conflict.
1. Try to disable other plugins, swith to default theme and check if issue repeats.
2. As a workaround, try to understand by which parameter scales reorder and just add some numbers to them: name or slug.
