it is hidden in the readme file however you just include [LINKEXCHANGE] in any post or page to create the form
all you have to do is place this in your theme:

this problem is caused by the fact that this script is the one which shows the menu and is included in the page footer thus if the page footer is not included then this script is not present so no menu
