I don’t know if it helps, but……….
I was having the same problem a couple of months ago (also was using a gmail account to receive the forms data)…
Found out that the e-mails were recognized as spam by the gmail anti-spam filter and also the e-mail used to send/receive the data…
So, i advice you to check our spam folder, and sigest your visitors to add the “form e-mail to their contacts”.
I know it’s not the perfect solution, but solved my problems until i’ve got a new corporate e-mail to be “the form’s e-mail”.

Just exclude the ‘image=icon’ parameter
