This can be closed – I was in a position where I was able to just reinstall my WP to clean everything out. Once I did that and reinstalled, everything is working as expected.
Not sure what happened from yesterday to today but after trying again today it worked this time.
The inquiry does not go to an email. If you look in your dashboard under subscribers (left hand side) you will see the emails that have been collected from the plugin.
This plugin clearly states that it does not send out notifications but rather just collects data. If you want to send out notifications when you publish a post then you will need to do so manually.
