Yes, that is the WooCommerce plugin.
The page the forminator form is not inside of woo pages. The Woo pages are totally separate. In trying to determine the problem, I was deactivating plugins one-by-one and when I deactivated that plugin, the PayPal buttons displayed.

I spent all afternoon trying to resolve it. Then I found this post and tried the new blog in Firefox. Problem solved.
