I have also been having this issue 🙁 It deletes my custom download template any time I update the plugin.

i tried uninstalling the plugin and installing a couple older versions 2.xx of the paid store locator with no luck getting it to work at all browsers 🙁
We are running wordpress 3.3.2 and don’t want to upgrade to 3.4 quite yet because some of our other plugins aren’t compatible yet.
please advise.
