As an example, you would need to change:
‘<p><label>’.__(‘First Name’).'</label> <input type=”text” name=”fnm” class=”input”/></p>’
to
‘<p><input type=”text” name=”fnm” class=”input” placeholder=”First Name”/></p>’
Note that placeholders are not supported in all browsers.
We do not recommend making such changes if you are not comfortable with HTML and PHP, since any errors you make while editing can cause you website to not load. Please make any changes at your own risk (and responsibility).

Sorry to hear about the issue, but the plugin is compatible with the latest WP release (4.5.3), we have it running on a number of sites. There may be a different issue on your site, perhaps the theme or another plugin has an error that is cascading down to this plugin. You may need to check your system logs to narrow the issue down.

Hi kolstef12! You can add as many labels for counties or states as you need, however there will be a point where the number of labels would become unmanageable for your users, if you anticipate having to add all the counties in multiple states.
Hi kolstef12! You would just keep adding labels and search boxes for other states, but beyond a certain number of states/counties it may just not be viable to use our system to run the directory, given that it is not really built to search by county.

You cannot have the form “displayed in buttons”. If you are unfamiliar with HTML we would not recommend attempting to perform advanced customization of the signup form.
It may be easier to just create separate signup pages for each membership level, then linking to the appropriate signup page from a membership level selection page.
Hi kolstef12! There is no way to remove the cards from the directory/search.
