Is there a special piece of CSS I must add for the mobile responsive version of the plugin. I am using it on this site http://testsite.thegreatmedicine.com/, but the mobile version does not scale down.
Is this something you can help me with please?

Hi mrlennyman, this plugin does not contain any CSS, it just outputs a simple HTML form, so it is up to your theme CSS to make the form responsive. You may want to contact the developers of your theme for assistance.
