Thanks for your reply. I used your instructions and got it to work. Is it possible to have the email show up as coming from a name of a company or a person instead of wordpress@wildlandfire2016.ca?

I have been having the same issues with attachments working on and off.

I am having a couple of issues. The file upload feature is not working. Sometimes the file is attached to the email and sometimes it is not.
Another issue is that it is not collecting all the data in the submissions database in the wordpress site server.
I also cannot get the email address of the person submitting the form to show up.
Frustrated.
