I found the problem, if Bad Query Strings is checked.

fails to load which make Appearance -> Menus to malfunction.
the images is just an example. like the Contact Form 7 plugin i will just insert this code [contact-form 1 "Contact form 1"] then in a specific page it will display a contact form. i would like to know how to do that.
