Thank you, I have just deactivated that feature. The form is currently showing. I will keep checking and report back here if I have any more issues.

I just checked and the form is once again not displaying. I did enable “Load form using AJAX” as suggested before and “Prevent page caching on form pages”. What is the next step for troubleshooting?
Thank you for your response. I just looked at the site again and now the form is showing, but I haven’t touched anything or made any of your suggested changes. I don’t trust that the issue is resolved, though, because it seems to go in and out on its own for reasons I don’t understand yet. Now that it is showing properly on the site, do you still see the display:none?
