I found the issue.
The parent page is marked as private. Tree view will allow me to create the page but wordpress will not save the page if the parent is marked as private.
I used 2.0.3 and I am in the same boat. After migrating content from http://www.domain.com to dev.domain.com, I am now forwarded to http://dev.domain.com/wp-signup.php?new=dev.domain.com
I saved my permalinks twice as requested, no results.
