What is the contents of the responsive-menu folder as it is saying that the autoload.php file doesnt exist.
It maybe that the download didnt complete properly.

I tried on iPhone 6 and a tablet and didnt see the issue.
Display: none wont do anything except hide them from view, as soon as that is removed it will go back to how it was previously.
Let me know what their theme support say.

I have just tried this on a few devices and am unable to replicate the issue.
What device and browser are you having this issue with? I have tried numerous times and it all works well for me.
Have you made sure to clear your caches and does the issue go away if you disable my plugin?

Can you try turning off the external scripts option quickly as it seems that the CSS has none of those settings in it.
Do you have any caching plugins installed that havent been cleared?

What version are you using as that seems to be the old 2.8 version shortcode.
The reason I ask is that there may actually be a way in the new version that allows the button to be skipped but Ive never tried it before!

In regards the header bar are you talking about the pro feature as the free version doesnt come with a header bar built in. If that is the case you will need to email me as I am not allowed to provide pro support on these forums.
In regards multiple menus on the same page this can be done by using the free and pro plugins side by side but not using just one version.
Alternatively you can create a third WordPress menu in the backend that combines the two and just use that.
