Whilst I appreciate volunteer support is what it says, repeating answers given two years ago for the same question verbatum is repeating standard replies. By the law of averages if you keep repeating the same answer to a similar question you must get some right.
My car does not go, put petrol in it, as a reply will work most times!

Fault diagnosis in the human body, something about as complex as WordPress it seems, is done by asking questions about the symtoms then drawing conclusions and recommending a solution. Not repeating past prescriptions.

Given the problem appears to be the link between the media list and the images, that have sucessfully been uploaded, some guidance on what tables hold what might shed more light on the issue?

Looks like the best bet is to strip it all down and start again.

Specifically I have tried the Twenty Eleven theme with plugins disabled and upload problem exists.
Clearly the use of ‘any theme’ was confusing.
Uploading was working with desired theme until upgrade then uploading problems started.
    Go Figure.
Clearly not the only one with this problem.
Perhaps someone, forum or WordPress, could investigate or ask more specfic installation questions to try and reproduce the problem rather than just repeat standard replies which have not worked for me or others posting same issue.

Resetting the plugins folder as in post reply via PhpMyAdmin did not work.

As posted – I was set to blank and I have overridden it as well.
    No change.

Images in 5 sizes have been uploaded and can be found in the uploads folder in either the year/month location or if Settings/Media ‘Organize my’ tick box is unticked in the uploads folder.

I have the same issue on 3.4.2 since upgrade
Images in 5 sizes have been uploaded and can be found in the uploads folder in either the year/month location or if Settings/Media ‘Organize my’ tick box is unticked in the uploads folder.
File permissions on all folders and files are all OK.
Using a browser and the url shows the image with no problems

    Any other ideas??
